Notice of Privacy Practices

This Notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully. This Notice is a draft and must be reviewed by your HIPAA compliance counsel before publication.

We may use and disclose your protected health information (PHI) for treatment, payment, and health care operations. We will obtain your written authorization for any other use or disclosure not described in this Notice or otherwise permitted by law.

Treatment: we may use your PHI to provide and coordinate your care. Payment: we may use your PHI to bill and collect from your insurance plan. Operations: we may use your PHI for internal practice activities such as quality assessment.

We are required by law to disclose PHI in limited circumstances, including: imminent risk of harm to self or others; suspected child or elder abuse; certain court orders; and reporting required by public health authorities.

You have the right to: request a copy of your records; request amendments to your records; receive an accounting of disclosures; request restrictions on certain uses and disclosures; request confidential communications; and receive a paper copy of this Notice.

If you believe your privacy rights have been violated, you may file a complaint with us at the contact information below or with the U.S. Department of Health and Human Services Office for Civil Rights. You will not be retaliated against for filing a complaint.

We reserve the right to change this Notice and to make the new Notice provisions effective for all PHI we maintain. We will post a copy of the current Notice in our office and on this website.
